.03 Requirements to Renew License
F A. A pharmacist licensed in another state
and actively engaged in the practice of pharmacy whose Maryland license
expired (2) years or more before the date of filing a renewal
application may seek license renewal upon:
G (1) Submitting to the Board notarized
affidavits, or other evidence satisfactory to the Board, documenting the
applicant=s pharmacy experience during the (2) years immediately preceding the date of
the applicant=s renewal application.
Comments:
G (2) Submitting to the Board satisfactory
proof of licensure and good standing from all states in which the applicant is
currently licensed; and

(3) Passing Part II (MPJE) of the licensure
examination.
F B. A pharmacist not actively
engaged in the practice of pharmacy in another state whose license
expired more than 2 years but less than 5 years before the date of
filing the pharmacist=s renewal application may renew the license upon
passing:
G (1) The Practice of Pharmacy Reinstatement
Examination
G (2) Part II (MPJE) of the licensure
examination; and
G (3) Part III (Lab) of the
licensure examination
F C. A pharmacist not actively
engaged in the practice of pharmacy in another state whose license
expired more than 5 years before the date of filing a renewal
application may renew the license upon:
G (1) Performing 1,000 hours of service in a
community or hospital pharmacy under the supervision of a licensed pharmacist;
G (2) Submitting evidence satisfactory to the
Board documenting the pharmacist=s
1,000 hours of pharmacy service;
G (3) Passing the Practice of Pharmacy Reinstatement
Examination; and
G (4) Passing Parts II (MPJE) and Part III
(Lab) of the licensure examination.
.04 Waiver
For
reasons the Board considers sufficient, the Board at its discretion, may waive
any of the requirements set forth in Regulation .03 in reinstating a pharmacist=s expired license.
